WHAT TO EXPECT:
On your first visit to our office, we will carefully review your medical history, do a pelvic exam, take a urine sample for analysis and do a preliminary test called uroflowmetry. We have three providers who may see you at your visits; the physician, our Physician Assistant, or our Nurse Practitioner. The provider will talk with you about her findings, make recommendations for a treatment plan and schedule additional testing if necessary. Additional testing may include Urodynamics and/or Cystoscopy.
As a new patient, we ask that you arrive 20-30 minutes prior to your appointment time. This will allow time for you to complete the registration process and complete or sign any unfinished forms. You should complete the forms found below prior to your first visit when possible. Once your appointment is scheduled, and you have been given a user id and password, you may also complete these forms within our Secure Patient Portal and submit them directly. These forms will help our providers to better assess your condition and recommend a treatment plan specific to your individual concerns. This information is very important. Please take the time to carefully complete them to the best of your ability. If you have problems downloading or accessing these forms, please call 803-457-7000 Option 1, to have the forms mailed to you.
